Small percentage of clients served are adults (over 21) with disabilities. Eligibility is based on need for service. Senior Companions must be age 55 or older, meet an income guideline, and be willing to serve a minimum of 15 hours a week and to reach out and provide service to others. Senior Companions are paid a tax-free stipend for their volunteer work and mileage.

To contact Long Term Services and Supports, please call Dakota at Home. Home Care Services: Provides services to help older adults or adults with disabilities stay in their home based on their needs. Services include home care and shore services, personal care services, adult companion, and nursing services. Safety and Help Moving Around: Provides services in the home to help keep an aging adult or adult with a disability safe, secure, and independent. Services include medical equipment, medical supplies, and assistive devices. Nutrition and Meals: Provides healthy meals, nutrition screenings, assessments, education, and counseling. Senior Health Information and Insurance Education (SHIINE): Provides individuals information about the benefits they can receive through Medicare, and how to avoid fraud to protect their benefits. Caregiver Support: Offers caregivers the resources and tools they need to care for their loved ones and gives them an opportunity to take a break from caregiving or receive a stipend for providing care to a family member. These services also provide aging adults and adults with disabilities time to socialize and receive support in a community setting through Adult Day Services. Residential Living Services: Provides services and supports in assisted living centers, community living homes, and nursing facilities. Ombudsman Program: Provides advocacy, education, and information for residents, family, and staff in long term care settings. Adult Protective Services: Provides interventions to protect vulnerable adults from abuse, neglect, and exploitation.

The Home and Community-Based Options and Person-Centered Excellence (HOPE) waiver provides home and community-based services to people who need nursing facility level of care. The waiver allows people age 65 and older, and people age 18 and older who have a qualifying disability, to live at home or in the setting that is most independent for them. Services may include in-home services, respite and caregiver services, nutrition and meals, safety and help getting around, adult day services, community living home, and assisted living. The waiver is provided through Long Term Service and Supports.
