Fill out application at Falls Community Health during their business hours (8:00am - 9:00pm, Monday and 8:00am - 5:00pm, Tuesday - Friday). If approved, eye exams are scheduled any time between 1:00pm - 4:00pm (CT), on the 1st or 3rd Wednesday of the month.

Walk-in to fill out paperwork to qualify. Must complete application in person at Falls Community Health.
Eligibility
Must have ID, with Sioux Falls residency. Must have no insurance (no Medicaid, no Medicare, nothing through the Exchange). For children under 16, parents must call Sioux Falls Family Vision 605-275-6100 to discuss options.
