Homeless Birth Certificate Waiver | South Dakota Department of Health
Provides, at no cost, a certified copy of a person's South Dakota birth certificate upon application and proof of homelessness. This is a one-time benefit. Individuals must complete and submit both the Vital Records Request form and the Affidavit of Homeless Status (must be signed in front of a notary with both required parties present) for a Certified Copy of a Birth Certificate. The standard application requirements apply and each applicant must provide applicable identification as stated on the application.

Application process
Affidavit of Homeless Status for a Certified Copy of a Birth Certificate must be signed in front of a notary public by both the individual experiencing homelessness and the Homeless Services Provider (HSP) with knowledge of the individual's housing status. An affidavit may not be accepted as proof of homelessness unless completely filled out and signed by both required parties.
